This form is for customers needing an instrument appraisal for insurance purposes. This service is intended for fully working instruments to add to a policy. This service is not authorized for insurance claim after damage or loss. This document must be dated, and proof of ownership and current condition must be confirmed via the form below.

We will send you a PDF to submit to your insurance company which includes the replacement value of your horn. The insurance replacement value is not the same as the resale value, if you are looking to sell your instrument.
